Beyaz İnci Ağız ve Diş Sağlığı Polikliniği acts as data controller under Turkish Law No. 6698 on the Protection of Personal Data ("KVKK"). Address: Orhangazi Mahallesi İstiklal Caddesi No: 115/3 Esenyalı - Pendik / İstanbul.
2. Personal Data Processed
Data submitted via the contact form — including name, phone number, e-mail address, treatment enquiry and preferred appointment time — is processed. Health data is processed only with your explicit consent.
3. Purpose of Processing
Your data is processed to handle appointment requests, provide information about treatments and fulfil legal obligations under Articles 5 and 6 of KVKK.
4. Transfer of Personal Data
Your data may be shared with domestic service providers, including appointment management and communication infrastructure providers. Transfer of data abroad is carried out only with your explicit consent.
5. Method of Collection and Legal Basis
Personal data is collected via the website contact forms, telephone calls and WhatsApp messaging. Legal basis: explicit consent (KVKK Art. 5/1) and performance of a contract (KVKK Art. 5/2-c).
6. Rights of the Data Subject
Under Article 11 of KVKK you have the right to: learn whether your data is being processed; request information if it is; learn the purpose and whether it is used accordingly; know third parties to whom data is transferred; request correction of incomplete or incorrect data; request deletion or destruction; request notification of third parties; object to outcomes arising from automated analysis; and claim compensation for damage.
7. How to Apply
To exercise your rights, you may deliver a written application to our clinic address in person or send it from your registered e-mail address. Applications are responded to within 30 days.
